For this week, you will develop a leadership case narrative to share with the class for discussion. For this assignment, you should carry out the following steps:

Develop a clinical or non-clinical case narrative (story, or case study) related to leadership and teamwork.

This will be most useful if the case is informed by some real practice experience. For example, you might consider a situation you witnessed or were involved in that involved some need for leadership.

Be sure to include pertinent information such as: What was the situation about, in what context did it occur, who was involved, what actions were taken, and how did things play out?

Your goal here is to provide a framework for others in the class to discuss the case and the leadership that occurred – or did not occur.

After reviewing the various Module 4 Resources related to care coordination., give some thought to the coordination of care in your current practice setting and explore the following questions:

• What (if any) resources/technology are used in your practice setting to facilitate coordination of care?

• How and when do healthcare professionals communicate with one another?

• Are social workers, nurses, or other healthcare professionals available to help promote continuity of care?

• How does the nurse-provider relationship influence coordination of care?

• What role does staffing play in promoting coordination of care?

• What are the primary barriers for providing high quality coordination of care in your setting?

• What opportunities to improve coordination of care can you identify and how might you help promote change?
