Describe how your selected program theory is being used to establish the program.
Goals and objectives/ Logic Model (Chapter 6)
Using SMART goals/objectives, create a logic model that outlines your plan, complete with anticipated outcomes.
Program Implementation (Chapters 7 & 8)
Define the activities/strategies, staffing needs, and estimated budget needed to execute the program
Provide an estimated timeline for the completion of each activity.
